To scope a new model of multidisciplinary clinical support for younger people with long term neurological conditions living in nursing homes in the West Midlands - A pilot project to explore the knowledge, skills and experience of staff that care for younger people with long term neurological conditions (LTNC) in nursing homes (NH’s), and develop a business case for the provision of a specialist team to provide support on an ongoing basis, providing early intervention and advice aiming to reduce harm and maintain optimal health for residents.
